PLEASE NOTE: Effective October 2010, the Board stopped producing the Bimonthly Disciplinary Report. The Sept-Oct 2010 edition is the last one produced. The Bimonthly Report, archived below, is a record of all Board actions taken during the previous eight weeks. This includes disciplinary actions such as revocations, suspensions and disciplinary consent orders, as well as non-disciplinary actions such as reentry agreements. The report includes the licensees name, location and license number along with a brief summary of the cause of action and the final action taken by the Board.
