A fee of $247.50 U.S. dollars is to be paid at the time the application is submitted ($200.00 for the license application fee, $38.00 for the criminal background check fee and $9.50 for the NPDB/HIPDB reports).  Fees must be paid by VISA or MasterCard.  Fees received are not refundable.
