All PLLCs are required to file organization papers with the NC Secretary of State. Prior to filing, however, the state requires PLLCs to receive certification from the NC Medical Board that the PLLC meets all requirements for such businesses to practice medicine in North Carolina.

1. Articles of Organization ( one copy)

2. A signed and notarized PLLC 1 form—Certificate of Organizing Members and Application for a Certificate of Registration for a Professional Limited Liability Company for the Practice of Medicine (The PLLC. 1 form may be downloaded completed and filed or used as a recommended format)

3. Check for $50.00 payable to NC Medical Board

4.  Business Address form
