a. Report of the Medical Board actions regarding perfusionists from the November Board Meeting. (See Attachment 2) | For information only

4. Licenses Issued
a. Perfusionist licenses issued since the last PAC meeting. | For information only

5. Changes to Education Certification form
a. The Medical School Certification form has been changed on the MD/DO application.  In order to be consistent with the other applications, the Education Certification form in the perfusionist application will need to be revised. (See Attachment 4) |  Staff Recommendation:  Approve changes to the Education Certification form.

6. ABCP Recertification Process
a. Discussion regarding the ABCP recertification process & the possibility of PAC requirements for a minimal number of cases reported during biennial renewal.  (See Attachment 3) | Staff Recommendation:
